Program Overview: Sustainability Plan
The program's impact on utilization of inpatient, emergency department, obstetrical services and primary care clinics, is carefully analyzed and compared with baseline and historical data. This analysis defines approximately how much the "system" (hospitals and clinics in the patient's community) saves, or avoids spending, by funding the Camino de Salud Network care coordination services.
Sustainability is enhanced through two major outlets:
- A gradual shifting of the case management functions to the community clinics, as they build internal capacity to provide more in-depth case management and disease management services for appropriate patients. This allows the system to leverage clinic-specific resources, such as Medicaid wrap-around payments to clinics that are Federally Qualified Health Centers (FQHC's), and further identifies the patient with the clinic as the primary care home.
An extensive prenatal and interconception referral process by which community clinics and LAC+USC Healthcare Network co-manage care. This allows LAC+USC Healthcare Network to leverage delivery resources and the clinics can remain sustainable by receiving MediCal resources which can support continuous care for the entire family from the referral of the baby back to the community clinics |
