Program Overview: Core Strategies

The Camino de Salud Network utilizes three core strategies to ensure that the highest quality and culturally sensitive care is provided to the patients and families with emphasis on improving health outcomes.

1.   Clinic to Hospital Communication with Centralized Care Management System:
The communication between the partnering clinics and LAC+USC is the key to improving patient flow through the healthcare system. LAC+USC Healthcare Network has provided the ability for the community clinics to access the Affinity electronic medical records system which allows for case notes, medical records, lab results and other medical information to be available to partnering clinics.  

In addition, all clinics have access to the COPE Health Solutions/World Doc Care Management Web-Accessed Software.   This innovative web-based system provides a shared comprehensive care management framework on which clinic, hospital and other care management staff can track patient appointments, disease-specific "report cards" and disease registries, social and medical "gap" tools with automatic referral making and tracking mechanisms, and electronic Comprehensive Perinatal Services Program (CPSP) paperwork that can be electronically populated and then printed for paper charts.

2.   Primary Care Home in the Patient's Neighborhood:
The clinics and who will partner in this network provide accessible, multi-disciplinary primary care in a community based setting, including both basic medical care and social support needs (multidisciplinary care center) that are culturally and linguistically appropriate. This "medical home" philosophy is a crucial component to the overall theme of the Camino de Salud Network.

3.   Centralized, Shared High Quality Patient Medical and Support Resources:
A unique feature of the Camino de Salud Network is that it closely coordinates the patient's care between the hospital and the clinics, while allowing a sharing of high-cost intensive resources. As part of the overall LAC+USC Healthcare Network strategy, re-orientation and re-positioning of specialty care and diagnostic resources based on geographic locations, will improve patient access. The Camino de Salud Network has integrated Community Health Specialist and/or CPSP provides help patients navigate through the health care system, access appropriate social resources such as hosing and reinforce positive health and lifestyle behaviors
