Is the Camino de Salud Network Sustainable?

The program’s impact on enrolled patients, particularly utilization of inpatient, emergency department and primary care clinic services, is carefully analyzed and compared with baseline and historical data.  This analysis defines approximately how much the “system” (hospitals and clinics in the patient’s community) saves, or avoids spending, by funding the Camino de Salud Network care coordination services.

Sustainability is enhanced through a gradual shifting of the case management functions to the community clinics, as they build internal capacity to provide more in-depth case management and disease management services for appropriate patients. This allows the system to leverage clinic-specific resources, such as Medicaid wrap-around payments to clinics that are Federally Qualified Health Centers (FQHC's), and further identifies the patient with the clinic as the primary care home.
